Medication Calculations, Opioid Conversions and Titration: A Practice Review

Objectives:

  • Develop a patient-specific pain treatment plan to reduce in pain intensity while minimizing side effects
  • Demonstrate the use of an equianalgesic dosing table
  • Identify the pharmacologic, pharmacokinetic, and cost differences of various opioids and dosage forms
  • Initiate opioid therapy or convert between different opioids, dosage formulations, or routes of administration in an actual or simulated patient

Presenter Bio:

Natalee Felten, PharmD, BCPS is the Education Manager and Call Quality Coordinator at Enclara Pharmacia. Natalee is responsible for overseeing new hire pharmacist training, pharmacy student internship activities, and the call quality program, as well as participating in the drug information service. Natalee previously served as a collaborative practice pharmacist and clinical pharmacist. She has over 12 years of experience as a pharmacist, including 11 years in hospice pharmacy, in addition to clinical roles in hospital and in community pharmacy.

Throughout Natalee’s career, she has been continuously involved in the education and training of healthcare professionals and students, including an appointment as an adjunct professor at Drexel University College of Nursing and Health Professions and University of the Sciences in Philadelphia. Natalee earned her doctor of pharmacy degree at Wilkes University, Nesbitt School of Pharmacy. She completed a 1-year post-graduate pharmacy practice residency program at Hahnemann University Hospital and maintains certification as a Board Certified Pharmacotherapy Specialist (BCPS).
