  3. Establish goals of treatment and regularly re-evaluate with patient and caregivers

Presenter Bio:

Erin Kennedy is an Enclara Pharmacia clinical pharmacist and a member of the Drug information team. She is responsible for recommending appropriate pain and symptom management therapies based on patient specific data and providing real-time collaboration with nurses to create cost-effective medication care plans. Erin is also responsible for responding to internal drug information requests that often result in client education. She earned her doctor of pharmacy degree from University of Iowa College of Pharmacy and maintains certification as a Board of Certified Pharmacotherapy Specialist (BCPS) and a Certified Geriatric Pharmacist (CGP).
