For urgent symptom management, medications are available from our Neighborhood Connections pharmacies or, if prescribed, from a previously dispensed Enclara Pharmacia ComfortPak. Appropriate planning and assessment are encouraged to reduce time to palliation and promote positive patient and family experience.

Our formulary Management Utilization Guidelines (MUGs®) include both evidenced- and experienced-based guidelines, with more than 50 symptoms and conditions relating to symptom management of the patient’s terminal diagnosis. Pharmacists are available to contact prescribers to discuss appropriate medication selection.
Often times, controlled substance prescriptions represent a challenge for a hospice to obtain in an efficient, safe and timely manner. We facilitate this process and ensure all prescription requirements are met.
Our Mail Order pharmacies are certified to receive electronic prescriptions including controlled substance prescriptions, providing a convenient and secure method for prescribers. Prescribers may also access our portal for electronic prescribing applications that integrate hospice specific patient, formulary and pharmacy information.
